This article describes errored syslog caused by PSM(Power Supply Module) failure.
Affected platforms: MX10004, MX10008, MX10016, PTX10004, PTX10008, PTX10016.

Following syslog generated and "PSM Not OK" alarm raised.

"d2d" is short for DC(Direct Current) to DC converter.
 

There are two d2d converters inside a PSM module: d2da and d2db.
Either converter failed will trigger "PSM <slot> Not OK" alarm as well as errored syslog as above.
 

Please contact JTAC for further troubleshooting.
In case of hardware failure, an RMA is needed to replace the affected PSM.
